Health Savings Accounts (HSA) - Guide/Enrollment
A Broker/Agent’s Step-by-Step Guide to Establishing a Blue Healthcare Bank Savings Account for Members
Prerequisite
- Confirm that your individual client has applied for and been approved
for BlueCross BlueShield HSA-eligible high-deductible health plan (HDHP).
Application Process
All forms for enrolling and maintaining your client’s HSA are
available at www.bluehealthcarebank.com or by calling (800)
663-BLUE.
Step 1: Broker forms
Step 2: Individual and Family Plan enrollment
- Provide the individual with a Blue Healthcare Bank enrollment
packet. Encourage your client to carefully read the entire booklet,
including every form, so he or she understands how his or her HSA
will be administered.
- Ask the individual to complete and sign the application.
- Collect the application from the individual. Make sure your Agent
ID number is on the
application and that the application is complete, and send them to
Blue Healthcare Bank at least two weeks before the plan’s effective
date. This will ensure that individuals receive the welcome kit and
debit cards in a timely manner.

Step 3: Individual confirmation process
- The individual will receive a Welcome Kit mailed to his or
her home. This kit includes website information, starter checks
and deposit slips, and the toll-free customer service number.
The individual will also receive a complete copy of the Truth in
Savings Disclosure with Schedule of Fees, and Deposit Account
Agreement, which includes Funds Availability.
- Debit Card information will be mailed to the individual's
home separately from the Welcome Kit.
- The individual will receive a monthly statement showing the
account balance and transaction history.
Step 4: Ongoing support
Our customer services representatives are available from 8:00 a.m.
- 9:00 p.m. Eastern Time, Monday through Friday to assist you or your clients with
any questions.
